While waiting in the barn for Chloe, Clark is attacked by a Phantom Zone prisoner. When he comes to it, he is in an asylum being treated by Dr. Hudson. Clark is told that everything related to his alien heritage has been made up in his head. He can’t even use his powers. Only a fellow inmate named John believes him, claiming he himself is an alien from Mars. Clark manages to escape from the asylum. He immediately returns to the Kent farm to find Lana living in the barn loft. Lana says that after Jonathan Kent died, Martha sold the farm and married Lionel Luthor.

Clark pays Martha a visit at the Luthor mansion. Martha wants her son to return to the asylum so he can be treated for his mental illness. When he refuses to comply, she calls Lionel’s security to forcefully take him. Clark manages to evade the security. He is surprised to find Chloe waiting in a getaway ride outside. Unlike everyone else, Chloe believes that Clark is an alien.

Still convinced that Lex is behind everything, Clark pays Lex a visit in Metropolis. He discovers that he caused Lex to loose his legs when he failed to save him on the bridge a few years back. After returning to Chloe, Lionel’s security catches up with Clark. It turns out Chloe believes Clark is an alien because she spent time in the same asylum. Chloe dies protecting Clark. Clark winds up back at the asylum. While in his cell, Clark overhears John, the patient who believes he is an alien, tell him that he has been trapped in an alternate reality by a Phantom Zone prisoner. John has entered Clark’s mind to help him defeat the prisoner, but it’s up to Clark to take the first step in driving him out.

In the end, Clark must choose whether to live out a dream life with Lana in this alternate reality or fight off Dr. Hudson, who is the form the Phantom Zone prisoner has taken, and return to the real world. Despite the temptation to be with Lana, Clark fights off Hudson, allowing John to suck the Phantom Zone prisoner out of Clark. As John flies away, he turns into the same martian-like creature that helped Clark defeat another Phantom Zone prisoner in Seattle, proving John and the creature are one in the same.
